“Population Health Management” for Student Success
Increasingly, today’s students may lack the background and support network needed to help them anticipate and meet the challenges of college. Progressive institutions are adapting to these changes by re-organizing advising to create a more scalable and effective student support model. This framework, known as Population Health Management (PHM), is inspired by proven principles in the health care industry. In this session, PHM principles will be discussed along with examples of how some innovative colleges and universities are shifting to PHM-like practices.
